Percy Place – Complex Care Home
This home is designed to accommodate adults who have a serious and persistent mental illness in addition to other medical and/or behavioural complexities. Occupants will benefit from a structured, safe, and supportive environment and receive 24 hour care from both Registered Practical Nurses and Personal Support Workers. Case Management and Peer Support services are also connected to this home. A client- directed approach to care allows clients to maintain housing stability and tenure while participating as partners in their care to the level they are capable of.
Lakeshore Home – Complex Care Home
This home provides care for individuals living with complex medical and Psychiatric conditions who may also have Acquired Brain Injury, mental health or addiction issues, resulting in risk of homelessness and frequent involvement with Police, Withdrawal Management and Emergency Services. Residents benefit from an open and safe environment that fosters a culture of transparency, growth and inclusiveness. Services provided include on-site Nursing, Personal Support Workers, and other supports to wrap services around the individuals and keep them safe in the community. A client directed approach allows clients to maintain housing stability and tenure while participating as partners in their care to the level they are capable of.
The Transformational Housing Program is a barrier free home located within the community of North Bay that can accommodate eight adults who are living with a serious mental illness, or an acquired brain injury, and who remain relatively independent in their day to day lives. Occupants are provided with a structured, safe and supportive environment so that they can live and maintain their independence. The home is staffed by Peer Support Workers 8 hours a day, 7 days a week, who assist occupants in meeting their needs, as directed in their Service plan. This includes participating in life skills development, as well as broadening knowledge of community resources.
